Can a well-established school be granted provisional accreditation with conditions?

Can a well-established school be granted 6 months provisional accreditation with conditions? Yes.  Accreditation decisions are not informed by how well established the school is but by the relevance and sufficiency of evidence provided by schools to support their compliance with provisional accreditation criteria. For instance, some fairly new schools get it right the first time they submit their portfolios of evidence.
